    Call our office and speak with a member of our front office staff. They will be happy to schedule your appointment. New patients can learn more about each of our physicians and the services they provide on this website.

    There are Health Questionnaires to fill out before New Patient visits, Annual Well Woman visits, Menopause Consultations and Sexual Health Consultations. These Questionnaires can be downloaded and printed from this website so that you can fill them out ahead of time. If you are not able to download the appropriate forms let our staff know that and they will send you the forms by mail.

     

    In addition there is a Patient Registration Form and Patient Message Authorization that need to be updated regularly.

    Appointment Cancellations

    If you are unable to keep your appointment please call in advance to cancel it. Your courteous call to cancel an appointment will allow another patient to be seen. Cancellations should be made at least one work day in advance. Not showing up for an appointment without a cancellation call will result in a $45 "No Show" fee.

    You will receive a courtesy reminder call 1-2 days before your appointment. In addition you will receive a reminder by mail when your Annual is due.

  • Telephone Calls

    Telephone Calls

    We welcome your telephone calls to our office when you have a question, a problem or need to be seen. Please make non-emergency calls between 9:00 -12:00 and 2:00 – 5:00. Our receptionist will schedule an appointment for you if you request one, or she will direct your call or question to your physician or her medical assistant. Your call will be returned by the end of the day or within 24 hrs.

  • After Hours CAll Coverage

    After Hours Call Coverage

    If you call the office with an urgent problem at night or on the weekend, you will reach our answering service. Please identify who your doctor is and briefly describe what the problem is. The answering service will contact your doctor and your doctor will return your call.

     

    For life- threatening emergencies, patients are urged to call 911 to be taken directly to the hospital emergency room. The Emergency Room physician may then call your doctor.

  • Prescription Refills

    Prescription Refills

    When you need a refill on an existing prescription, call your pharmacy at least 3 days before you need the medication. The pharmacists will fill the prescription or if there are no refills left they will eSend or fax us the request Your doctor will review your medical record and either approve the refill or call you to set up an office visit. If you have a request for a prescription medication not previously prescribed by your current Avenues For Health doctor, please us call during routine office hours.

  • Medical Records

    Medical Records

    Electronic Health Records are strictly confidential and will be released in whole or in part upon your written request. Records will be printed and mailed to you or available for pickup within ten business days of receipt of your request, unless an urgency exists which requires they be sent sooner. Printing records takes time and materials and so there is a $35 fee to copy records.
